Medical data
Data about your child's health is special personal data, and we treat it with the utmost care.
When your child becomes a client at Praktijk voor kinderfysiotherapie Eindhoven Noord we will enter your personal data into our administration and your paediatric physiotherapist will create a file. This is a legal obligation. At your first visit, we are required to verify your child's Burgerservicenummer (BSN) based on a valid identity document. However, no copy of this will be made.
Your child's file contains information that is necessary for a careful assessment of his or her state of health and information about the examinations and treatments carried out.
We exchange information with other healthcare providers, such as general practitioners, doctors at child healthcare centres, hospitals, etc. We do this via secure systems and only if necessary and with your knowledge. Our staff will only consult your details if this is necessary for the performance of their duties.
Praktijk voor kinderfysiotherapie Eindhoven Noord adheres to the legal retention period for medical files of 20 years after the end of treatment.

Your rights
You are entitled to see your data. If it turns out that information about you is incorrect, you are entitled to have it corrected or removed by us. It may not be possible to (fully) comply with a request (e.g. if your inspection leads to a breach of privacy of others). If you would like to see your data, please contact your treating paediatric physiotherapist or our secretary.
Complaints
If you have a complaint about the processing of your personal data, please contact us directly. If we are unable to resolve the matter with you, we are naturally very disappointed. You always have the right to submit a complaint to the Dutch Data Protection Authority, this is the supervisory authority in the field of privacy protection.
